    Tuesday, May 26, 2026
    You must complete and submit the request for exclusion no later than Tuesday, May 26, 2026 by emailing the form, submitting the form online, or mailing with a postmark date no later than May 26, 2026.

  • Please Note, the United States Postal Service (USPS) postmarks no longer reflect the date an item is deposited in a local mailbox. Instead, USPS postmarks are added to mail when the mail is processed at a regional USPS facility. To ensure that you have submitted your exclusion request by May 26, 2026, place it in the mail well in advance of that deadline or bring it to a USPS facility before May 26 and obtain a free manual postmark at a USPS retail counter.
